Real KSM parsing

The server reads the machine-file structure and stitch commands. Renaming another file to .ksm is rejected.

Short-lived processing

Viewer uploads are streamed to the restricted engine and are not added to conversion history or free-job usage.

KSM compatibility: KSM uses needle-change commands and cannot preserve full thread-colour metadata.
